Sympathy Announcement Form

If you lost an immediate family member and like to announce that through a blast-email to all District employees, please fill out and submit this online form. Please allow at least two (2) business days to process your request.
Unlike obituaries on the newspapers and websites, District's sympathy email announcements are brief and contain the minimum and necessary information only.

Who can submit this form?
- A District employee who lost an immediate family member.
- Or a work colleague authorized to provide the information on his/her behalf.

Who can be considered an Immediate Family Member?
Someone's spouse, parent, grandparent, children, grandchildren, adopted children, step-parents/step-children, brother, sister, aunt, uncle, mother in law, father in law, brother in law, sister in law, daughter in law, sons in law.
